At Charles, Fenchel, and Associates, we employ a Care Manager to offer our elderly clients the broadest range of options to deal with their concerns and problems. Our clients have the benefit of not only the traditional legal mechanisms that may be required, but also have the option of consulting a Care Manager to address broader concerns and topics from a social service perspective.

Our Care Manager has the knowledge and experience to evaluate an individual's needs for out of town family members, as well as for those who live nearby but are not aware of how to utilize the appropriate local services. Additionally, the Care Manager can evaluate a client’s functional abilities, home environment, and family and social support systems in order to assist individuals to continue to live independently, or to assess the need for alternative living arrangements such as supportive housing, assisted living facilities, or long term care.

Care Management Services can be utilized to assist with a single, specific task such as helping you find a daily caregiver or as a way to oversee the entire caregiving process.  In addition to helping seniors and their families directly, our Care Manager can act as a liaison with other professionals in the elder care network.

A consultation with our Care Manager involves an in-depth assessment that evaluates the situation, identifies solutions, and works with you to design a plan of care for maximizing your elderly loved one’s independence and well being.
